Performance Evaluation Form
Staff & Individual Contributor
A recommended form, which can be modified, for staff and individual contributor annual job evaluations. This form was created by a system-wide Talent Management Performance Evaluation subcommittee using best practices from multiple campuses and current research in the HR field. You are free to modify it or customize it to meet your campus needs. The first two pages can be used alone as a goal-focused review or all five pages can be used for an in-depth review focusing on job competencies and related knowledge, skills and abilities.

This form was created by a system-wide Talent Management Performance Evaluation subcommittee based on a Luoma Leadership Academy Action Team’s recommendation and using ideas from multiple campuses and current research in the HR field. You are free to modify it or customize it to meet your campus needs.
